Position Summary

Primary Responsibilities

  • Assist with bathing, dressing, grooming, toileting, continence care.
  • Provide mobility assistance, transfers, and fall prevention.
  • Prepare meals and assist with feeding if needed.
  • Perform light housekeeping tasks.
  • Provide companionship and emotional support.
  • Report changes in condition to the office immediately.
  • Follow the care plan and document tasks completed.

Qualifications

  • Active CNA or HHA certification.
  • Experience with seniors preferred.
  • Reliable, compassionate, and dependable.

Position Summary
The LPN provides direct nursing care under the supervision of an RN. Performs skilled tasks, monitors client conditions, and ensures safe, effective care.

Primary Responsibilities

  • Provide wound care, medication administration, catheter care, injections.
  • Monitor and report changes in client condition.
  • Follow RN-established care plans.
  • Document all nursing care accurately.
  • Communicate with supervising RN.

Qualifications

  • Active LPN license in Massachusetts.
  • One year experience preferred.
  • CPR certification.

Position Summary
The Registered Nurse (RN) provides skilled nursing care to clients in their homes, ensuring high-quality, safe, and compassionate care. The RN completes assessments, develops care plans, supervises home health aides, and coordinates care with physicians, families, and the agency.

Primary Responsibilities

  • Conduct comprehensive nursing assessments including SOC, recerts, and reassessments.
  • Develop and update individualized care plans.
  • Provide treatments, wound care, IV therapy, and skilled nursing tasks.
  • Monitor and document client health; report significant changes.
  • Educate clients and families on medications and disease management.
  • Communicate with physicians and interdisciplinary team.
  • Supervise Home Health Aides/HHA.
  • Maintain accurate documentation.

Qualifications

  • Active RN license in Massachusetts.
  • Minimum one year clinical experience (home care preferred).
  • CPR certification.
